The space shuttle Challenger exploded shortly after liftoff on January 28, 1986. All seven crew members were killed in the explosion, among them a school teacher who was part of NASA's Teacher-in-Space program. The disaster prompted President Ronald Reagan to create for the Presidential Commission on the Space Shuttle Challenger Accident. The commission concluded that a combination of mechanical failure and human error caused the explosion.
